• Your office or apartment must be currently serviced by ZOOTS in order to use this form to sign up for service.
• Complete the information requested to begin service and put it in a ZOOTS bag with your items to be cleaned.
• Leave your bag in your designated drop location before 9:30 a.m. to be picked up.
